eForms are an essential tool within OSCAR Pro, allowing users to create, save, and fax electronic requisitions and forms.
If a clinic has PDF they would like to add as an eForm in OSCAR Pro, it must be created or added using one of the approved methods below.
- Upload a .zip file of an existing eForm into OSCAR (instructions below, obtained from our eForm repository)
- Create their own eForms using the eForm Generator
- Contact support to create a custom eForm (separate fees may apply)
⚠️ Important - PDFs Cannot Be Uploaded Directly
Uploading a PDF file that has simply been saved into a
.zipfile will not create a usable eForm.Zipping a PDF does not convert it into an eForm.
PDFs must either:Be built using the eForm Generator, or
Be downloaded as an existing eForm that has already been created for OSCAR Pro
If this step is skipped, the form will not function correctly in the system.
Step 1: Search the eForm Repository
Access the eForm Repository using the link below:

Use the search function to locate your desired form.
Use the Download button to download the .zip file of the eForm.
Please note, all eForms in the repository are available for download at no cost and are provided on an as-is basis. Any changes or development requests on eForms found in the repository may be associated with a cost.

Upload eForm to OSCAR:
Once you have obtained the
.zipfile of the eForm, follow the steps below to upload it into OSCAR Pro.Ensure the file is a downloaded eForm
.zip(not a zipped PDF)Go to Administration > Forms/eForms > Manage eForms

- Click the Import item at the top

- Click Choose File and select the eForm .zip file from the local drive

- Click the Import button to proceed
- You should see a message indicating this was successful. Sometimes there is an import message, depending on the message you can select Import below the message, it may still load, if it does not it is likely that you already have this form.
- If you have identified that you do not have this eForm and you are still unable to load please create a ticket for Support to review the eForm for you. Be sure to include the name of the new eForm as well as the ZIP file attachment so we can investigate.
