Here we're going to go over how to Delete older eForms from your eForm Library, and how to restore any accidentally deleted eForms. As well as how to rename them, and export copies.

To access the eForm Library, we're going to navigate to Administration > eForms > eForm Library:
How To Delete And Restore eForms:
From here you're able to search with the Search bar located on the far right:
This can help filter out the same copies of similar eForms.
In the example above, we can see that the older PHO General Form is still available, but the newest version has also been added to the eForm Library. To ensure the older version is not selected while in the patient's chart, we're going to Delete the older version.
To Delete an eForm, we're going to click on the trash can icon under Actions on the far right:
This will remove it from your eForm Library.
The eForms that have been deleted, can be restored or viewed by clicking on View Deleted from your eForm Library:
From the Deleted section, there is a Restore button located on the far right under Actions that can be used to add any eForm back to your eForm Library:
The Restored eForm would now be active in your eForm Library.
How To Edit Name:
The name of the eForm can be modified by clicking on the pencil icon under the Actions column:
This will load all the HTML code for the eForm, and the name can be located at the top to be modified:
Renaming the eForm can make it easier to locate, for instance if it was obtained from the Repository and it has the full clinic name written out (William Osler Health System), but at the clinic it is always referred to in a short abbreviation (WOHS), this will be easier to locate if referencing common language in the clinic.
Additionally, you can rename with numbers or * in the front, which will place the eForm in the top position when accessing the eForm Library within the patient's eChart. This is beneficial for requisitions that are frequently used on a daily bases.
The Additional Information box can be modified to advise on a version, or if it has signature stamp, or if it is populating current user information, versus doctor/mrp information.
The Save will be located at the bottom of the page:
Export A Copy:
A copy of the eForm can be exported by clicking on the download icon under Actions:
This will save a zip copy of the developed eForm to your computer. This could then be shared with another clinic, or if you work at multiple clinics and are looking to have the same eForm available on the other OSCAR Pro system.
In order to add it to another eForm Library at a different clinic, you would need to navigate to Administration > Manage eForms and click on Import:
You would then be prompted to upload the desired zip file. Once imported it would then be available in the eForm Library.
Please note, that if it is a custom eForm please ensure that you have permission from the Account Holder to share it with another location.
