In this article we will review how to create a Consultation Request to another practitioner.
From within the patient eChart, scroll to the bottom left side, the purple banner marked Consultations.
Select the + to open the module.
We are going to break this page up for review.
Starting with the upper portion, the referring practitioner will populate based on the MRP on the patient chart. There is a drop down if this needs to be edited.
You need to select the service you are referring out to. Once this is selected you can select the Practitioner (Consultant).

In my screenshot below I have selected Cardiology and WELLstar, Health Technologies. Once I selected the Consultant, their contact information also populated.
Next let's look at the left hand side panel. You will see Go To Widget, by selecting this you have the opportunity to attach any additional documentation that is needed to send along with the referral.
This will redirect you to the Attachment Manager located at the bottom of the window:

Each document, lab etc listed will have an eye on the far right so you can click in order to preview to insure you are selecting the correct item. Check the boxes of each document to select.
When you are finished with selection, click the Save button.
You can confirm these are attached as they will now be listed below Selected Attachments.
The next area to review is the letter itself, this area you will type in the reason for referral. There are 2 additional called Pertinent Clinical information and Significant concurrent problems, you can free type in this area as well. Or, if you have information within the patient CPP, you can click the buttons to have this information populate from there.

Any current Medications and Allergies will auto populate.
The last part you'll see is the Note and Signature, you can add anything additionally in the notes. Your signature will auto populate if you have one set.

Submit and Fax
The last part of this process is to send the consultation to the referral practitioners office.
These options are at the bottom and top of the Consultation form.

BOTTOM
Submit and Print - Though lesser used this will save and allow you to print off the consultation.
Submit Consultation Request - this will save the consultation. you want to use this if the admins staff are responsible for faxing out.
Submit and Fax - this option will do just that submit (save) the Consultation and Fax it off.
*When you fax the consultation off, you should update the Status to Pending Specialist callback.
