Here we will be reviewing a few of the modules within the patient's eChart on the Ophthalmology system.
CPP Entries:
The CPP boxes include:
- Current History
- Diagnostic Notes
- Medical History
- Past Ocular History
- Family History
- Ocular Meds
- Other Meds
To add a new entry to one of the modules, we would need to click the + for that particular module to open a text dialogue box:
Within the text box an entry can be manually typed in, a date can be set, as well as the position selected for how it will show in the eChart:
The blue arrow and white pad of paper needs to be clicked to Save the entry and close the window!
Please note the scissors icon can be used to Archive an entry, and the closing door icon can be used to close the window without making a new entry.
The date of entry will be displayed beside the entry for quick visible reference:
This feature can be disabled by provider under their Preferences and opening the Eyeform eChart Preferences:
Please note that Preferences are specific to each Provider's login, so anyone that wishes to make a change will need to update their own Preferences when logged into the EMR.
Patient Log:
This is an area that you can enter in quick notes to be referenced for the patient. A new entry can be created in the same manner as the CPP notes, we want to click on the + beside that module to load the text box:
The Eyeform now supports a fixed-height Patient Log with a scrollbar, helping keep the eChart shorter and easier to navigate when patients have many log entries.
This reduces unnecessary scrolling and makes the chart easier for clinicians and staff to review during daily use.
This feature can also be disabled from a provider's Preference.
Documents, Diagrams and Photos:
These modules allow the upload of of different document types to be stored in one area to be easily referenced.
Documents can be uploaded by click on the + beside the module and completing the requesting prompts:
We need to:
- Select Type - referral, legal, lab, insurance, other, etc..
- Enter Title of the Document, which will be visible from the eChart as the name
- Choose File to locate the document from your computer
- Add to upload the document
Diagrams is an area where you're able to review any saved eForms for the patient. New eForms can be added from the eForm module on the right hand side.
From Diagrams, we would click on the header name Diagrams to open the new window and click Show All to load the saved eForms:
Photos can be added similarly to Document, but the Type that would assigned would all need to be Photos to be visible in this module:
Photo will automatically load as the Type and the Title.
Appointment History:
This area will list all scheduled appointments for the patient, including any future appointments.
When we click on the header Appointment History it will load a new window of the patient's appointment history:
When we click on an actual date listed, that will generate a PDF document that will download to your computer. This PDF will list all entries made to the eChart during that scheduled appointment:
This can be used as a quick reference, or provided to patient regarding encounter.
Examination History:
This module allows you to load a window with all the details for any examinations that have been completed for the patient.
We need to click on the header of the module Examination History and then select the tests on the left and click the arrows to populate on the right side:
After all the tests have been selected, the Submit button would need to clicked to load the Examination History window.
Please note that you can select multiple items on the left with the Shift key and your up and down arrow keys on your keyboard.
In the Examination History window, you can filter it by entering a certain date range at the top.
Billing History:
This module provides quick access to the patient's previously billed invoices, as well as a short cut to create new invoices.
When the header, Billing History is clicked a window will load with all of the patient's Billing History:
From the Billing History window, you can edit invoices, and search for billed service codes.
When we click on the + for this module, a window will load to create a new invoice for the patient:
VA IOP:
This module stores all entries that have been entered for a Vision Assessment and IOP:
Please note the measurements cannot be opened from the VA IOP module, but you can click on the header in the centre to expand those measurements.
Ocular Procedure:
This module is located on the right hand side of the eChart and will list any procedures completed.
The previous Ocular Procedures can be viewed in a new window with notes entered by clicking on the header Ocular Procedures:
By clicking the + for this module a new window will load to enter in a new Ocular Procedure:
Once the information has been entered, we would click Submit to save!
Macro:
The intention of the Macro is to allow the Practitioner to create a template/tickler and billing in one with specific impressions. This can also include Ocular Procedures.
To create a new Macro the + needs to be clicked and the Add on the new window, you can check out this video HERE.
To launch a Macro, we would click on the name of the Macro listed under this module:
Exam Template:
The Exam Template feature in the patient eChart allows providers to quickly and efficiently apply commonly used measurements to an exam. Templates can include partial or complete exam findings and can be reused for any patient.
Exam Templates can be created from any patient’s eChart and then applied universally.
To create a new one, we would click the + beside the module to launch the new window and click Add:
To use an exam template, it must be clicked from the eChart under the Exam Template module:
