Here we will review how to enter in an Impression Plan and an Arrange Plan for an ophthalmology appointment.
Impression Plan:
An Impression Plan is a summary of the encounter with the patient, including the physician's diagnosis of the patient. This may include the severity of the diagnosis and a clinical interpretation of the findings from the appointment and tests completed.
The Impression Plan is located at the bottom of the patient's eChart:
This box will lead with appointment details, such as date and time and the reason.
Notes can be free hand typed into the text box, or templates can be utilized to aid in the task.
Templates:
Above the Impression box is a drop down of Templates that can be referenced:
They can be searched on the left hand side to populate into the Impression note:
Templates can be customized with the option New / Edit Template from the Templates drop down.
Please note, Templates are clinic wide not provider specific. If creating a new Template the name could contain the provider's initials to ensure when searched that they're selecting their personal template. For example LM - Glaucoma.
Impression History:
For returning patients, there is an Impression History button that will load a new window to review past Impression notes:
This works as a great quick reference for past clinical assessments.
For more details on past visits, the Appointment History can be opened from the left hand side of the patient's eChart:
When an appointment is clicked it will generate a PDF document that can be opened to view all details that were entered in the chart for that visit:
This works as a reference tool for past visits.
Arrange Plan:
Part of the Impression will include arranging next steps to manage the condition.
The button Arrange Plan below the Impression box will open to a new window allowing you to quickly enter in tasks that need to be completed:
This allows you to set clear directives for next appointment and any additional tests that need to be booked.
In the Arrange Plan window the option to Add can be done below any that apply for the patient:

Book Procedure
Book Diagnostics
For each option the information can be completed (routine/ASAP, name of tests and comments) according to the patient's condition.
Once completed, the Submit button can be clicked to Save!
The Arrange Plan details will then be displayed below the Impression:
If you wish to include these details in the Impression Plan, you can click Generate Note:
In order to assign these tasks to a staff member, their name can be selected from the drop down and click Send Tickler:

Signing Impression:
An Impression note will have an internal save draft that is completed every few seconds:
If you're exiting the eChart prior to Signing off the Impression note, it is recommended to use the Save icon:
The Impression note can then be completed the next time you open the eChart.
When the Impression note is completed, there are 3 Signing options available, the best one to use would depend on your personal workflow.
The first icon, with the white note pad and blue arrow will Sign and Save the Impression note, and close the eChart window for you.
The second icon, the pens and pencil will complete a Verify and Sign, this can be used to verify another provider's entry in the eChart, such as for a Resident or Student.
Finally, the last icon will complete a Sign, Save and Bill which will open the Invoice Creation page after signing the Impression note and closing the eChart window.
Macro:
Both the Impression Plan and Arrange Plan can be executed by applying a preset Macro within the patient eChart.
Macro is located on the left hand side, and new ones can be created by clicking the + for that module:

By clicking on an available Macro, it will then be applied to the patient's chart.
Please note, Billing can also be set within a Macro. This means that an Invoice would be generated when the chart is closed after applying the Macro.
