Here we will review how to enter in measurements within the eChart for Ophthalmology. We will review this for a new patient and for a follow up appointment.
Accessing eChart:
This can be done a number of ways, such as using the E short cut from a scheduled appointment, or on a patient search:
Now that we have accessed the eChart, lets begin entering in measurements!
New Patient:
In the centre of the Encounter Chart will be the Eye Measurements:
This area will load collapsed, but there are options for Expanding/Collapsing in the top and bottom right hand side.
The option to save any measurements entered will be located in the top and bottom left hand side.
Let's expand all and review the different sections!
Glasses Hx:
The top section is for Glasses, the Type of Glasses can be selected from the drop down:
The values can be entered in accordingly, and on the right side is a calendar icon to aid in entering the date.
If required, you can click ADD to display a second line:
Vision Assessment and Vision Measurement:
Measurements can be entered if the appropriate tests are being completed for the appointment.
If measurements have been entered in a different section of the Measurements, you can use the Copy/Paste links to enter the values instead of manually entering in them.
There is also a Clear link available to quickly remove all values entered if a mistake was evident.
Remaining Measurements:
The remaining sections can be entered if required.
Please note that for some areas there is a Normal link available, in addition to the Clear link:
Normal will automatically paste in normal results.
Once completed, click the Save Measurements located on the left side, either top or bottom of the Measurements section.
Returning Patient:
After opening the patient's eChart, the previous measurements will be shown when expanded.
For Glasses Hx, there is a link to view old glasses quickly:
A new window will load displaying a search option, as well as listed past entries for a quick reference.
In addition to reviewing/entering in new values a date must be entered on the right side.
We want to ensure that we are updating the measurements, even if there has been no change since the last visit.
Placing the cursor in a measurement box will update it from a grey box to a white box, this indicates that it is newly entered for the new date/appointment.
You may tab forward on your keyboard to quickly update the fields to new.
The copy/paste/clear/normal links are still available to be utilized.
Vision Assessment and IOP:
If a vision assessment and IOP has been entered previously, it is also available under the VA IOP module on the left hand side of the chart:
Please note the measurements cannot be opened from the VA IOP module, but you can click on the header in the centre to expand those measurements.
Once completed, click Save Measurements to update any changes!
Exam Templates:
For new and returning patients, there is also the option of applying an Exam Template to automatically enter in the measurement values. Exam Templates can be created at clinic level to assist with this task.
To apply an Exam Template it must be selected from the module on the left side:
To create a new Exam Template, click on the + beside the module.
