Accessing Create Demographic
First let's look at accessing the screen to create a new patient demographic.
If you were creating an appointment and searching for the patient name, if no patient name matched what you input, you will have the opportunity to create a new demographic.
Note: To avoid creating duplicate patient records, if the patient has indicated they have previously been seen in your office you may additionally want to search inactive (highlighted above) patients as well.
You can also select Search to have the Create Demographic option. Search has 2 areas, the green banner on the dashboard and the Quick Search option.
From within your Unclaimed Documents in your Inbox, you also have the ability to Create New Demographic.
Mandatory Fields
- Patient Name
- Patient Date of Birth
- Patient Sex
- Health Insurance Number and HC Type (Province for HIN) are not mandatory BUT if you are registering a patient that does not have a health care number you must set the HC Type to Other.
Additional recommendation to add the Provider that will be their MRP for that patient. This will insure the provider is automatically tagged on any incoming labs or assigned documents.
While often very useful for contact, address and phone numbers are not mandatory in creating the patient demographic.
An additional area to note is where to add Referral Physician or Family Doctor if you are being referred to adding to this area will insure that it can populate on your billing.

Also, note the Alert note, this adds a booking alert that can assist as a reminder when the patient is booking their appointments.
