The OntarioMD's eForm integration allows users with active OneID credentials to submit an electronic form to a participating organization from your OSCAR Pro EMR.

Introduction
While named similarly to the native eForm functionality existing in OSCAR Pro, the OntarioMD eForm is a separate integration that requires setup and onboarding to utilize.

The eForm Portal currently supports form submission to the Ministry of Transportation.
As noted by OntarioMD:
Once the eForm is submitted from the patients eChart:
- Providers with an active HRM integration receive a copy of the submission into their Inbox
- The submission is routed (as an HRM) to the patients eChart
- Provider has the option to Print or save a copy of the submission to PDF
Accessing the eForm Portal for a Specific Patient
NOTE: In order to access the eForms integration from your EMR, the associated provided must have a working OneID account and configured to utilize eForms by Ontario Health and/or OntarioMD.
- Navigate to the eChart of the desired patient
- Locate the Provincial EHR Services section at the bottom left
- Click on the eForms link under Provincial EHR Services
- If prompted, enter your OneID credentials to authenticate
- Once successfully authenticated, you will be redirected to the eForms portal:
Submitting an eForm
- Click on a desired eForm category such as Ministry of Transportation
- Select desired eForm to complete
- Submit the associated fields and use the Next page to proceed through the various stages. In the example below, the eForm has 4 stages:
- Click Submit on the final step
- Optional: once the submission is complete, use the Print button to download a PDF copy of the submission.
- If the clinic maintains an active HRM Integration within your OSCAR Pro and you are set to receive HRM's, a validation of this submission will be sent via the HRM portal and route to both your Inbox and the patients eChart.
